In an emergency with a severe bleeding wound, the right first aid measures can save lives. The following will provide you with professional wound management knowledge to help you take the right action at the critical moment.
Basic principles of wound management
1. Expose the wound: If the wound is covered by clothing, the clothing should be carefully removed or cut open to expose the wound for better assessment and treatment.
2. Foreign body treatment: If there is a foreign body in the wound, do not try to remove it, as it may help stop the bleeding. Instead, pressure should be applied on both sides of the foreign body to push the wound edges together.
Emergency assistance and communication
Ask someone else present to call emergency services immediately and provide details of the wound location and extent of bleeding. If you are alone, use the hands-free speaker feature of your phone to maintain communication with ambulance control while treating the casualty.
Hemostasis and pressure control
In the absence of a sterile dressing tourniquet, a clean, non-fluffy cloth, such as a towel, handkerchief, or clothing, can be used to apply direct pressure to the wound. Make sure the cloth used is as clean as possible to reduce the risk of infection. Keep the pressure on the wound, but not so tight that it restricts blood circulation. Check the blood circulation outside the bandage by pressing the nail or skin outside the bandage for five seconds until the color turns white, then release the pressure. If the color does not return within two seconds, it means that the bandage is too tight. For heavy bleeding in the head and limbs, digital arterial hemostasis can be used. This method uses fingers to compress the artery proximal to the wound, and directly press with the fingers or palm for 5 to 10 minutes. This method is suitable for bleeding from small arteries, veins, and capillaries. Press it on the deep bones to occlude the blood vessels and interrupt the blood flow, thereby stopping the bleeding.
Preventing shock
Loss of blood may cause the person to go into shock. Help them lie on a rug or blanket, and lift and support their legs above heart level. Loosen tight clothing around their neck, chest, and waist, and cover the person with a blanket to keep them warm.
Continuous monitoring and CPR preparation
Continue to monitor the casualty's level of responsiveness until help arrives. If they become unresponsive at any time, be prepared to begin CPR.
