In clinical practice, various types of wounds are often delayed due to infection, which not only affects the patient's medical experience, but also brings great pressure to clinical medical staff. Many postoperative wounds are not well cared for, which greatly increases the risk of incision infection.
Here is a case of postoperative wound infection:
Ms. Ge , 39 years old, gave birth to a second child . On January 6, 2022, a hysterectomy was performed, and the incision was not healed well after the operation. On February 18, 2022, after the surgery was disinfected with iodophor, it was covered with a disposable wound dressing.
Wound Assessment:
On March 29, 2022, the patient complained of local traction pain. After removing the dressing, it can be seen that the incision still oozes blood and exudate, and 3 ulcers (the larger one is about 1.5 cm*1.5 cm*0.3 cm) have not healed.
Dressing after:
1. Disinfect the local wound and surrounding skin with iodophor, and let it dry.
2. Clean the wound with iodophor with a saline cotton ball.
3. According to the shape and size of the wound, cut the silver ion dressing and fix it on the wound, and then fix it with an ultra-thin hydrocolloid sticker.
Follow-up evaluation:
2022-04-04 The wound surface is clean, the granulation tissue grows well, and there is no obvious local oozing and exudation. Continue to change the dressing according to the above method.
Characteristics and application advantages of silver ion dressings and alginate dressings:
Silver ion dressings commonly used in clinic include silver ion dressing and alginate dressing.
Longterm Medical silver ion dressing: is a silver compound or nano silver dressing, which is mainly antibacterial by releasing silver ions or nano silver particles. The dressing is suitable for the coverage of moderate to severe exuding body surface wounds: it can be used for pressure ulcers, lower extremity ulcers, diabetic foot ulcers, second-degree burns, skin grafts and donor sites or trauma wounds. Compared with ordinary dressings, silver ion dressings It has a broad antibacterial spectrum and can effectively inhibit various pathogens, including Staphylococcus aureus, Pseudomonas aeruginosa and Streptococcus. At the same time, the dressing can slowly release silver ions for sterilization while absorbing the exudate, and the action time is longer. After the activity of the microbial cells is lost, the silver ions can be freed from the microbial cells to be sterilized again. In addition, silver ions are not easy to develop resistance to bacteria, so they will not cause mutation of bacteria.
Longterm Medical alginate dressing: is a natural polysaccharide carbohydrate extracted from seaweed, which is a natural cellulose. A functional wound dressing with high absorption properties composed of alginate .After the medical dressing contacts the wound exudate , it can form a soft gel to provide an ideal moist environment for wound healing, thereby promoting wound healing and relieving wound pain. The filling used was an alginate strip dressing for cavity wounds.
Three main points of wound dressing and wound treatment
1. Clean the wound:
When the necrotic tissue of the wound is small, the wound should be washed directly with normal saline, then disinfected with iodophor, and finally the wound should be cleaned again with normal saline. Pay attention to the cleanliness of the wound after cleaning.
2. Debridement:
It is also possible to use the wet dressing hydrogel to dissolve and remove necrotic tissue or scab for safe and effective autolytic debridement.
3. Control infection:
When there is less wound exudate, use silver ion dressing to reduce wound odor. At the same time, bacterial culture and drug susceptibility tests were performed on the wound surface, and sensitive antibiotics were selected according to the test results.
